1. Mar 2004 to date:
Sir Ganga Ram Hospital, New Delhi
I
was appointed Consultant Endocrinology in this prestigious 650 bed
multispeciality tertiary referral institution, after I took premature
retirement from the Army Medical Corps.
2. 2001- March

Indian Army in June 2001. The Endocrine department of this hospital
comprises a 20 bed indoor endocrine facility and it runs one
of the busiest referral endocrine outpatient service, managing
the whole spectrum of endocrine disorders possible. The department
is equipped with a state-of-the-art Radioimmunoassay laboratory,
Hologic DEXA bone density laboratory and a Color Doppler laboratory
for peripheral vascular disease related to diabetes.
3. 1998
to 2001: Indian Naval Hospital ‘Asvini’, Mumbai
The premier hospital
of the Indian Navy, located at Colaba, Mumbai, is a multispeciality
750 bed tertiary care hospital. As head of the department of
Endocrinology I oversaw the indoor, outdoor and endocrine laboratory
service. It provided me an exposure to the practice of clinical
& laboratory endocrinology in the teeming metropolis of
Mumbai.
4. 1994
to 1997: Command Hospital Calcutta
This
large multispeciality 700 bed hospital, catering to the medical

career in Endocrinology.
5. 1992

The Endocrinology

Diabetes
Mellitus: ‘Niche Areas’
With the diabetes
epidemic sweeping across the world it is imperative for health
care professionals to focus on diabetes management to tap the
potential of growth in the hospital clientele. With advances
in technology and the introduction of continuous glucose monitoring
devices and insulin pumps for insulin delivery, setting up diabetes
care centers would prove to be good economics for the hospital.
Establishing a
dedicated Diabetes Research Center/clinic, could
provide:
- Comprehensive advanced diabetes
care.
- Demand management in diabetes
care.
- Network diabetes care.
- Outcomes research in diabetes
care.
- Establish ‘Benchmarks’ for
diabetes care in the country.
- Assist development of ‘National
Policy on Diabetes Care’.
- Preventive diabetology based
on lifestyle changes.
Osteoporosis
and Bone Health
With the advent
of Bone mineral density testing using state of the art DEXA
scanning the problem of Osteoporosis has caught the attention
of the medical world. In combination with vitamin D deficiency
it is a burgeoning public health problem of immense proportions.
Establishing an Osteoporosis Research Center/Osteoporosis
Clinic to manage osteoporosis, both medically and surgically
("minimally invasive" surgery for the management of
acute fractures), would prove to be a futuristic innovation.
Women’s
Health Issues
With increased
longevity, a greater number of women are living past the menopause.
Infact a third of a woman’s life is postmenopausal. The controversies
and the heat and dust generated by the publication of the study
results of the Women’s Health Initiative & the Million Women
study regarding hormone replacement therapy are cause for concern.
In this context establishing Women’s Health Clinics
– for post menopausal health management would seem prudent.
Thyroid disorders
Establishing Thyroid
Care Clinics for managing thyroid disorders based on
evidence based structured format would go a long way in stream
lining thyroid care.
General Endocrine Clinic
. A general
Endocrinology clinic to cater to endocrine disorders such as
neuroendocrine, adrenal, pancreatic disorders, hyper-androgenism,
endocrine tumors, male and female reproductive endocrinology
and other miscellaneous disorders would help focus on hard core
general endocrinology.
Newer
Frontiers in Endocrinology: Cardiovascular Endocrinology
An important
new area of Endocrinology is Cardiovascular Endocrinology.
The Endocrinologist is uniquely skilled at diagnosing and
treating these cardiovascular endocrine disorders. The focus
of a Cardiovascular endocrinology clinic could include
- Endocrine causes of Hypertension
- Metabolic syndrome comprising
abdominal obesity, insulin resistance, hypertension, dyslipidemia
& glucose intolerance
- Obesity – A pandemic
- Endocrinologists making
great strides in management of Obesity
- Dyslipidemia – Endocrinologist
plays a specific role in complex cases
- Thyroid disorders and cardiovascular
disease
- Hypothyroid & hyperthyroid
patients have deleterious effects on the cardiovascular
system
- Cushing’s disease – especially
the subclinical subtle forms
- Diabetes and cardiovascular
disease
